Yes. Select “Request a test” and complete your information plus test and provider details. A patient test request is not a finalized laboratory order until reviewed by your healthcare provider.

Choose “Portal access when ready” and submit your profile. We will notify you when secure sign-in and result viewing are available.

Submissions are transmitted over HTTPS for request intake. Full HIPAA portal workflows—including encrypted storage and role-based access—activate when the patient portal launches.

Digital signatures for requisition forms will be collected in the secure patient portal, not on this interim request form.
